Your Appointment Information

Please bring your insurance card and referral from your primary medical doctor (if needed) to your appointment. If this is your first time at our office, or if we haven't seen you for a long time, we may need to dilate your pupils. As this may cause some blurry vision and light sensitivity you may wish to have someone accompany you
